You can declare that you have lost your tertiary qualification certificate by declaring in this form.

Statutory Declaration for Continuous Eligibility Req

UTC affirming a newly required Statutory Declaration (SD) for continuous
checks eligibility requirement biennially as a prerequisite for renewal of
registration.
